ABSTRACT

The syndrome differentiation of Yin and Yang has the function of controlling the other six principles in the eight principles syndrome differentiation, which is a higher level or general induction of the disease. In the clinical process of traditional Chinese medicine, syndrome differentiation of Yin and Yang runs through the whole process of disease diagnosis and treatment. For Parkinson's disease, syndrome differentiation of Yin and Yang is particularly important. Different symptoms, the transformation of pathogenesis during the development of the disease and the treatment of traditional Chinese and western medicine all reflect the characteristics of Yin and Yang opposition restriction, mutual root and mutual use,and the transformation of ebb and flow. This article discusses the background, application and value of Yin-Yang syndrome differentiation from three aspects:the origin and application of yin-yang syndrome differentiation, the basis of Parkinson's disease syndrome differentiation, and the status and role of Yin-Yang syndrome differentiation in Parkinson's disease. It is of great significance to guide the diagnosis and treatment of Parkinson's disease with "Yin-Yang as the key point".

ABSTRACT

Keratoconus is a progressive blinding eye disease that characterized by corneal thinning and protrusion, which accompanied with irregular astigmatism and impaired visual acuity. The irregular astigmatism of early keratoconus can be corrected by spectacles. For the irregular astigmatism of moderate to severe keratoconus, spectacles are no longer suitable, and contact lenses are the best choice for patients to restore vision. There are various types of contact lenses, making the selection very difficult. In addition, trying on lenses for a long time will increase the discomfort and overall feeling of patients, and greatly increase the workload of doctors. Thus, the article aims to summarize and discuss the classification of contact lenses, the application of contact lenses in different types of keratoconus, the complications of contact lens, and the current status and prospect of contact lenses, with a view to understanding the management and clinical application of contact lenses in keratoconus patients and to further improving the application value of contact lenses in keratoconus.

ABSTRACT

Danggui Sinitang is first recorded in the Treatise on Cold Damage written by ZHANG Zhongjing in the Han dynasty. It is composed of Angelicae Sinensis Radix, Cinnamomi Ramulus, Paeoniae Radix Alba, Asari Radix et Rhizoma, Glycyrrhizae Radix et Rhizoma, Tetrapanacis Medulla, and Jujubae Fructus and serves as a classic formula for treating the syndrome of blood deficiency and cold reversal. This study systematically reviews the records of Danggui Sinitang in ancient Chinese medicine books of various dynasties and the modern clinical applications to probe into the composition, plant species, processing, dosage, decocting method, and indications of Danggui Sinitang, aiming to provide a reference for the development and clinical application of this classic formula. The review of the records showed that there were a variety of records of Danggui Sinitang with different composition, and the composition of this formula listed in the Treatise on Cold Damage has a significant impact on later generations and has been used by medical practitioners throughout history. Although the dosage of some drugs decreased during the Ming and Qing dynasties, the medical practitioners continued to use the original formula. In terms of processing, although there were slight changes in the processing of Angelicae Sinensis Radix, Paeoniae Radix Alba, Glycyrrhizae Radix et Rhizoma, and Tetrapanacis Medulla, the original processing method was inherited. In terms of indications, Danggui Sinitang was designed to treat cold reversal due to blood deficiency and dysentery. Furthermore, it was used to treat headache, convulsive disease, infantile convulsion, and private part adduction in the Ming and Qing dynasties. Nowadays, this formula is mostly used to treat diabetes peripheral neuropathy, rheumatoid arthritis, dysmenorrhea, Raynaud's disease and other diseases. In terms of precautions, ancient physicians believed that Danggui Sinitang should not be taken by pregnant women and should only be used for limb chills caused by blood deficiency and cold coagulation. For limb chills caused by other reasons, this formula should not be used indiscriminately. Modern research has not reported any serious adverse reactions related to this formula. Danggui Sinitang has a definite therapeutic effect. In subsequent research and development, quality control standards of Danggui Sinitang should be established while its safety is ensured, and the related preparations should be developed and applied.

ABSTRACT

Neural recording electrodes enable the acquisition and collection of electrical signals from neu-rons,and these recorded neural electrical signals are an important means of understanding neuronal activity. As a major component of the brain-machine interface,neu-ral recording electrodes serve as a bridge between the nervous system and external devices. The extracted information can be used to understand the state of the brain and acts as a feedback signal to regulate external devices, thus providing important information for the clini-cal treatment of neurological diseases. Moreover, the electrodes can be used as a vehicle for drug injection to directly treat diseases.Since the time that Strumwas-ser used microwires to achieve long-term recordings of neural activity in hibernating squirrels, implantable elec-trode technology has gradually improved over three gen-erations of development, and progress has been made in improving the biocompatibility, mechanical performance(size, shape, density, etc.),and signal-to-noise ratio.Implantable neural recording electrodes can acquire sig-nals from cortical and deep neural clusters, with the advantages of high signal-to-noise ratio, information con-tent,and spatial/temporal resolution. However, there is still a need to improve the structure and performance of these electrodes;for example, their high invasiveness and lack of biocompatibility pose technical difficulties in the process of translation to the clinic. This paper reviews the basic requirements for electrodes, main recording methods and signal types, common types of implant-able neural recording electrodes, and their challenges and future development directions. With the continuous development of electrode materials, equipment, systems, and neurotechnology,it should be possible to apply neu-ral recording electrodes in clinical practice,to promote safe and efficient treatment of human diseases.

ABSTRACT

Lung cancer tops the disease list in the world due to the high incidence and mortality, and about 85% of lung cancer cases is non-small cell lung cancer (NSCLC). Most NSCLC patients are in the advanced stage at the time of diagnosis, with a low 5-year survival. Traditional Chinese medicine (TCM) plays a role in the comprehensive treatment of malignant tumors. Oral Chinese patent medicines, as an important part of TCM, have the advantages of stable preparations, mild taste, simple package, and accurate effective ingredients, which are different from decoctions. They have been widely used in the adjuvant treatment of NSCLC. In clinical practice, the combination of oral Chinese patent medicines with chemotherapy, targeted therapy, or radiotherapy, as well as the application of the oral Chinese patent medicines alone, can increase efficiency, reduce toxicity, prolong the survival time of patients, and improve the quality of life. The mechanisms of oral Chinese patent medicines in the treatment of NSCLC mainly include inhibiting the proliferation, invasion, and metastasis of lung cancer cells, promoting the apoptosis of lung cancer cells, inhibiting tumor neovascularization, reversing multidrug resistance, and regulating the immune functions, which reflects the multi-pathway and multi-target manner of TCM. The oral Chinese patent medicines commonly used in the clinical treatment of NSCLC include Jinfukang oral liquid, Shenyi capsules, Pingxiao capsules, Xiao'aiping tablets, Kanglaite capsules, compound Cantharis capsules, Huisheng oral liquid, Yangzheng Xiaoji capsules, Xihuang pills, Zilongjin tablets, and Cinobufagin capsules. There are many clinical and basic studies about the treatment of NSCLC with these medicines, while a systematic review remains to be carried out. Therefore, we systematically reviewed the mechanisms and clinical application of commonly used oral Chinese patent medicines in the adjuvant treatment of NSCLC, aiming to provide reference for follow-up research and clinical treatment.

ABSTRACT

Strengthening the standard formulation and quality management of traditional Chinese medicine(TCM) dispensing granules is an important part of the strategic planning for the development of TCM in China. In order to examine the clinical application and overall quality control of the existing national standards for TCM dispensing granules, this study classified and summarized the varieties in the existing standards, analyzed their clinical applicability, and discussed the characteristics of the test methods for identification, content determination and specific chromatogram/fingerprint. It was found that the coverage of the existing standards was inadequate in terms of quantity, and it was even weaker in the aspects of therapeutic efficacy, herb family, processing method and preparation method of TCM dispensing granules. It was concluded that the characteristics of national standards in test methods were summarized as follows:guided by clinical application, based on the reference system, taking specific chromatogram as a breakthrough, so as to improve the overall quality control of TCM dispensing granules. It is suggested that the coverage of national standards should be subsequently expanded to meet the needs of market development. In order to enhance clinical applicability, the content of national quality standards should be increased, including increasing variety diversity to meet the needs of clinical application, raising the standard requirements to improve the clinical medication experience, and strengthening effectiveness research to highlight clinical efficacy. At the same time, the accessibility of regulatory inspection is enhanced, the rules for the management of varieties without national standards are promulgated to lay the foundation for the healthy and orderly development of TCM dispening granule industry.

ABSTRACT

Through the textual research and analysis of the variety, origin, processing, quality evaluation and clinical application of Moslae Herba in ancient and modern literature, its origin of materia medica was clarified. Moslae Herba has experienced variety changes in history. Elsholtzia ciliata was the mainstream variety during and before the Song dynasty, however, during the Ming and Qing dynasties, emerging variety of Mosla chinensis rose to the mainstream status due to its remarkable efficacy and the formation of cultivation, and differentiated into two commodities(wild variety of Qingxiangru and cultivated variety of Jiangxiangru), cultivated products formed an authentic producing area in Jiangxi. The three varieties coexisted during the Ming and Qing dynasties, and the Elsholtzia varieties were gradually eliminated. Variety changes have caused changes in the functions and indications of drugs. E. ciliata had the effect of clearing heat and was mainly used to treat heatstroke and cholera, while M. chinensis was used for exogenous wind cold and dampness in the summer because of its warm and strong sweating properties, but not for cholera. Traditional Moslae Herba is mainly harvested in the summer and autumn (flowering to fruiting stage) and the above-ground parts are dry in the shade and used as medicine. Modern Qingxiangru is mostly harvested before the flowering period, and Jiangxiangru is harvested after flowering and fruiting in late summer and early autumn. In summary, according to the 2020 edition of Chinese Pharmacopoeia, the dried above-ground parts of Moslae Herba should be selected for Xinjia Xiangruyin in the Catalogue of Ancient Famous Classical Formulas(The First Batch), mainly the cultivated variety of Jiangxiangru, and the raw products is cut into segments and used as medicine. It is suggested that when applying and developing famous classical formulas containing Moslae Herba at different periods of time today, the origin should be established in conjunction with clinical efficacy.

ABSTRACT

Digestive tract diseases, especially digestive tract tumors, including liver cancer, pancreatic cancer, and colorectal cancer, have high incidence in China. Digestive tract tumor is one of the top 10 cancers in terms of the number of new cases and deaths in the world, and the incidence and mortality of tumor diseases have been increasing year by year. Therefore, the prevention and treatment of tumors is particularly important. With the application and promotion of traditional Chinese medicine in the medical field and the rapid development of molecular biology and pharmacology, more and more potential active components of Chinese medicinal materials have been extracted and studied. These active components can inhibit tumor cells in a multi-target and multi-pathway manner. Cinobufotalin is an effective component extracted from the skin of Bufo bufo gargarizans. It has been prepared into a variety of agents with anti-tumor, immunomodulatory, cardiac boosting, pain-easing, anti-inflammatory, and swelling-relieving activities. In clinical practice, cinobufotalin is mainly used to assist the treatment of liver cancer, lung cancer, colorectal cancer, gastric cancer and other malignant tumors, which can reduce the adverse reactions of patients in the middle and late stages and improve the quality of life and five-year survival rate of patients. The available studies of molecular mechanism have demonstrated that cinobufotalin can play a therapeutic role by inducing cell apoptosis, regulating cell cycle, inhibiting cell proliferation and angiogenesis, modulating immune response, reversing multidrug resistance, enhancing radiochemotherapy sensitivity, inhibiting tumor inflammation, invasion, and metastasis, etc. This review focuses on the clinical application and mechanism of cinobufotalin against digestive tract tumors in recent years, aiming to provide a theoretical basis for the anti-tumor research of cinobufotalin, promote the application of cinobufotalin in tumor treatment, and facilitate the further research and development of this compound.
